Summa Health at Home™ for Chronic Severe Illnesses
Our Summa Health at Home program can be invaluable for patients with chronic severe illnesses such as diabetes, hypertension, pneumonia, stroke, and heart failure, -or those at an increased risk for falls or medication errors. The program is particularly helpful for patients who find it difficult to leave their homes because of surgery, injury, or certain mental health issues such as depression or heightened anxiety. By receiving home healthcare, many patients avoid trips to the emergency department, admissions/readmissions to the hospital or admission to a nursing home.
