Social Work and Transitional Care

Social workers are critical team members to patient care at Summa Health. Sometimes it’s difficult to know what level of care a patient will need when leaving the hospital. How do you get connected to the resources and services you need?

Our highly experienced social work and transitional care team helps patients move from the hospital setting to the next level of care. They work with an interdisciplinary team to coordinate the complex planning necessary to ensure patients receive the care they need, such as home healthcare or support groups.
