What is the Complex Care Institute?
The Summa Health Complex Care Institute brings together complimentary components of healthcare to treat patients throughout the entire care continuum. This means an interdisciplinary team — comprised of specialists from palliative care, senior health, pastoral care, the Complex Care Clinic and pain stewardship — works collaboratively to provide patients a healthcare experience that is easier to navigate and takes care of the whole person.
The Institute incorporates clinical programs, whether they be in the hospital, outpatient or in the home, as well as community partnerships, education and research to provide comprehensive support. Our goal always is compassionate care that keeps patients and their loved ones top of mind.
Clinical Programs
Palliative Care
Palliative care is specialized medical care for people with serious illnesses. This type of care is focused on providing relief from symptoms and stress of the illness. The overall goal of palliative care is to improve the patient’s and the family’s quality of life while ill. This type of care is for people at any stage of serious illness, whether that illness is curable, chronic or life-threatening.
Senior Health
As people age, their healthcare needs change. Our team of specialists diagnose and treat a wide range of conditions that affect people as they age and help improve the overall quality of life for older adults.
Complex Care Clinic
At Summa Health, we recognize some patients have complex care needs such as medical, behavioral and social health challenges. Our highly trained team at Summa Health’s Complex Care Clinic has developed innovative ways to meet these needs and can provide care in the home, hospital and office to ensure the best possible coordination of care in all care settings. We also strive to make the healthcare system easier to navigate. Our goal is to improve patients’ independence and quality of life and to help reduce unnecessary trips to the emergency room and hospital.
Pastoral Care
When it comes to leading a healthy, productive and satisfying life, we know that the emotional, mental and spiritual aspects of good health are just as important as physical aspects of health. Our clinically trained team of chaplains are available to assist patients, loved ones and employees with managing the stress and strain of illness, grief and trauma that impacts health and wellness.
Pain Stewardship
Pain often is a part of patients’ lives, but it needs to be treated appropriately and safely. Our Pain Stewardship program aims to improve the overall quality and safety of pain management through the use of national guidelines for best practice. The team identifies opportunities for improving pain management through chart review and coordinates interventions to improve the safe use of opioids and other pain modalities.
