Your personal story can be a powerful inspiration to others, providing the courage to press forward when challenges become distressing.

If you chose to share your story publicly, your story will be shared in a few weeks in an online collection of Summa patient stories. The collection will be accessible from our website and social media platforms. NOTE: Any information you share in your story, such as any health information, doctors visited or treatments you've received will be public. No contact information will be shared on your story, only your name and first initial will appear on your story.

Here are a few pointers for making sure your story is heard:

  • Be Personal: This is your story, and should focus on your own life experience and beliefs.
  • Be Specific: The most powerful and memorable stories include specific, real-life examples.
  • Be Concise: There's no specific limit on the length of your story, but you should try to communicate the essence of your story as clearly and concisely as possible.

I give permission to share my story publicly on the Summa Health website, social media platforms and print collateral materials. My story may be edited for content and grammar as decided by Summa's Corporate Communications department. I further acknowledge any confidential medical information included and disclosed in my story will be public. I agree in order to assist scientific, treatment, educational, public relations and charitable goals and hereby waive any right to compensation for such uses. I hereby hold Summa Health and its subsidiaries harmless from and against any claim for injury or compensation resulting from the activities authorized by this consent.
