Social Work

The Summa Health Equity Center has licensed social workers that engage with patients and families on social and health and wellness issues. They help address and resolve social, financial and psychological problems related to conditions spanning the entire healthcare continuum. This may include:

• Helping patients and families understand the illness and its treatment options
• Diagnosing and treating underlying mental illnesses through crisis intervention, counseling and psychotherapy
• Coordinating funding for medications, medical equipment or other services
• Coordinating referrals to appropriate agencies for follow-up care
• Participating in patient-care conferences with a multidisciplinary team of healthcare providers
