MyChart Frequently Asked Questions

What is proxy access?

Proxy access and allows a parent (or guardian) to log into their personal MyChart account and then securely connect to information regarding their family member. 

Proxy Access to a Child's Account

  • If your child is age 0-11 you will then be granted full access to your child's MyChart record.
  • Once your child turns 12 years old, access to your child's MyChart account will be automatically reduced to limited access (due to privacy laws). The child can edit the parent's access level from within MyChart. You can also request access from the front desk or the child's care provider if the child consents.

  • Once your child turns 18, all access to your child's MyChart account ends (due to privacy laws). The Patient can grant parents access to their MyChart account by submitting a proxy request through MyChart. You can also request access from the front desk or the child's care provider if the child consents.

Consenting Adult to Adult Proxy Access

In the case of an adult Representative accessing a consenting adult's record, such as a spouse, we ask that the Patient provide the MyChart Proxy Access Authorization Form (the form includes your consent for this type of access).

In the case of an Adult Child or a Legal Guardian Requesting Proxy Access for an Adult

We ask that the Representative bring in the MyChart Proxy Access Authorization Form with proper identification and supporting legal documentation if required, such as a court order or durable power of attorney for health care (DPOA). 

May I request proxy access for more than one person?

Yes, you would follow the same process for each qualifying individual.

Can I ask questions regarding a family member from my MyChart account?

No. MyChart offers direct access to your personal health record and communicating about another individual's information would be placed in your health record. This information would not appear in the correct health record and could potentially jeopardize medical care.

Can my spouse and I share one MyChart account?

No. Due to the sensitive nature of medical information, each adult must sign and submit a Release of Information request and establish their own MyChart account.

Is a Proxy Representative responsible for a Patient's bills?

Proxy access does not make you responsible for the bill. The guarantor is the person responsible for paying the bill. You can find the guarantor name and account number on your statement.
