House Call Programs

At one time, many doctors made house calls. Patients appreciated the convenience and found great comfort and compassion in receiving healthcare services in the privacy of their homes.

House calls are particularly helpful for patients who cannot leave their homes because of chronic, severe illnesses such as diabetes, chronic obstructive pulmonary disease (COPD), stroke, chronic heart failure, or those who have physical or memory loss that limits the ability to get out of the house.

To better meet these patients’ needs, SummaCare and Summa Health are pleased to offer this time-honored service.

Your care team is led by a nurse practitioner (NP) who specializes in care of older adults and partners with your doctor. Your geriatrician is a medical doctor who has been specially trained in the aging process and focuses on treating older adults.

Your NP will make scheduled home visits and collaborate with your geriatrician regularly. Geriatricians make visits annually to discuss an overall care plan, goals of care and complex medical issues. The integrated team, including your NP, your geriatrician, a pharmacist, a registered nurse case manager and a social work meet weekly to review care plans for all patients in the program.

While this house call program is open to both SummaCare Secure and traditional Medicare patients, there are additional benefits available to patients enrolled in the SummaCare Secure health plan, including a SummaCare case manager who is available to help individuals access additional healthcare resources, and a licensed practicing pharmacist who will review medications and help identify any potential issues such as side effects.

The House Calls Program is also affordable. There are no out-of-pocket costs for house calls for SummaCare Secure patients. However, traditional Medicare patients are responsible for normal co-pays and deductibles, just as they are when they go in for a visit with their current doctor.
