Geriatric Consult Services

Inpatient geriatric consultation services are available whenever an older adult is hospitalized.

The Institute’s geriatricians collaborate with geriatric nurse practitioners to deliver inpatient geriatric medicine consultation services on all units. The consult service supports the Institute’s goal of helping all seniors admitted to Summa Health hospitals maintain function and independence.

If a primary care physician spots a potential problem that he/she believes warrants further investigation, a geriatric medicine inpatient consultation service team member visits the patient while he/she is still in the hospital. The team evaluates the patient’s situation and makes appropriate recommendations for follow-up care or services.

The team also provides transitional care management to ease the move from hospital to home for the patient, caregivers and family members.

Typically, a follow-up visit at the Summa Health Senior Health Center occurs after the patient’s discharge from the hospital to evaluate how well the care plan is working.
