Post-Acute & Long Term Care Services

Summa works very closely with many area nursing homes, skilled nursing units and assisted living facilities through our Care Coordination Network.  Many of our specially trained geriatricians and nurse practitioners consult in these facilities to provide the best care even after you have left the hospital.

Palliative consult services are also available at many of these area facilities. These services offer a holistic care model focused on giving patients relief from the symptoms, pain and stress of a serious illness—whatever the diagnosis—while working to improve the quality of life for both the patient and the family. Summa’s palliative team has conducted more than 800 consults in these types of facilities in the past five years.
