Summa Health Online Application – Initial Application and Recredentialing Application

The Summa Health online application is a web based tool that enables both new applicants and current providers an electronic method to enter provider data, request privileges, upload supporting documentation, and electronically sign off on required forms used in the application process.  

The application itself offers streamlined entry fields to populate multiple forms and is pre-populated with data from the Credentialing database. All credentialed practitioners including medical staff, advanced practice providers and allied health, who wish to be credentialed for or reappointed at a Summa Health facility must utilize the application.

The provider can also identify a credentialing delegate to assist in the application process. Both the provider and the delegate are each assigned a unique temporary password to the secure website application that can then be updated after accessing the application the first time.

Reappointment of Hospital Privileges – Your application will be pre-populated with your most current information from the Credentialing database. You can update any information that is not correct or out of date and upload any new documents needed to complete and submit your application.
 
If after submitting your application, a change/update is required to the privilege request, email CVO@summahealth.org or call 234.312.2100 to make a request for revision of privileges.  You will receive an email with a clickable link that allows you to log in, update the privileges, and complete the eSignature process.

  • Allied Health Credentials PolicyPlease read if you are an allied health provider who is not employed by Summa but who is seeking authorization to work in the Hospital.   • Medical Staff Credentials PolicyPlease read if you are applying for Summa Health System Medical Staff membership. Please contact the Medical Staff Office at 330.375.7672 if you have questions about the content of the document.

Summa Health Hospital Verification

Summa Health provides an online hospital affiliation verification website for providers credentialed through their Medical Staff Services.  The link below will assist you in obtaining the verifications that you need in a timely manner.  If you are unable to locate a provider in the online verification, please contact the Credentials Verification Office at 234.312.2100 or by email at CVO@summahealth.org
