The Summa Health online application is a web based tool that enables both new applicants and current providers an electronic method to enter provider data, request privileges, upload supporting documentation, and electronically sign off on required forms used in the application process.
The application itself offers streamlined entry fields to populate multiple forms and is pre-populated with data from the Credentialing database. All credentialed practitioners including medical staff, advanced practice providers and allied health, who wish to be credentialed for or reappointed at a Summa Health facility must utilize the application.
The provider can also identify a credentialing delegate to assist in the application process. Both the provider and the delegate are each assigned a unique temporary password to the secure website application that can then be updated after accessing the application the first time.
